American Sign Language Direct Support Professional - Residential Program - Mesa area

Home and Community Based Services Program Coordinator - Sierra Vista / Clifton area

The Program Coordinator is responsible for the management of the Home and Community Based Services through the implementation of the RISE Mission, Vision, and Values and in compliance with all regulations and policies.

REPORTS TO: Regional Director

SALARY: DOE

LOCATION: Services are based in the Sierra Vista and Clifton areas

MINIMUM QUALIFICATIONS:

  • High School diploma or GED and at least a combination of two (2) years supervising staff and experience working in a related field.
  • Familiarity with the State System for Developmental Disabilities Division.
  • Must have verbal and written communication skills adequate to the specific job.
  • Must be at least 21 years of age.
  • Bilingual (Spanish/English) preferred.
  • Must pass background clearance.
  • Ability to work flexible hours when needed.

ESSENTIAL JOB FUNCTIONS:

  • Provide management and support in a manner that meets and exceeds the expectation of RISE consumers.
  • Responsible to recruit, hire, coach, train and discipline direct support professionals.
  • Maintain necessary records, logs, and other documentation as required by the planning teams, RISE, and the state.
  • Attend all staff meetings and other mandatory training as required.
  • Complete payroll summaries for employees accurately and within designated time frames ensuring compliance with DOL regulations.
  • Demonstrate respect and promote choice for those whom we support; daily activities shall be based on the person's choice and plan's outcomes.
  • Promote the rights of people receiving support and contribute to an environment that is free of abuse, neglect and/or exploitation.
  • Establish partnerships with RISE consumers, i.e. supervisor, staff, each respective family / guardian and funding agencies.
  • In conjunction the planning team, assist clients in identifying their plan outcomes.
  • Evaluate the effectiveness of each person's plan on a monthly basis. If the plan is not meeting the person's needs, will coordinate a team meeting to reevaluate the plan and develop a solution.
  • Quickly and effectively respond to emergency situations; will be responsible for rotation of emergency phone in a monthly basis.

Licensed Pediatric Occupational Therapist - East Valley

The Early Intervention Program continues to grow. The Core Team is composed of the following disciplines: Occupational Therapists, Physical Therapists, Speech and Language Pathologists and Developmental Special Instructionists.

The Arizona Early Intervention Program provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them to build on their child's strengths, and to enhance and promote the child's growth, development and learning skills in their natural environment.

REPORTS TO: Director of Early Intervention Services
LOCATION: Services are provided in the East Valley (Mesa, Gilbert, Chandler, Tempe and East Phoenix).
SALARY: Competitive salary starting at $55 per hr.

THIS IS AN INDEPEDENT CONTRACT POSITION WITH OPTION OF BECOMING AN EMPLOYEE AND QUALIFY FOR BENEFITS.


MINIMUM QUALIFICATIONS:

  • Must possess a Bachelor's Degree in Occupational Therapy at time of contract offer.
  • Must Hold or be eligible to obtain Arizona State Licensure at time of contract offer. 
  • Hold all necessary credentials before reaching a contract.
  • Pediatric experience a plus
  • In-Home therapy experience.
  • Bilingual English/Spanish preferred.
  • Candidates must pass background clearance.
  • Travel to attend meetings/training, and to provide services in a natural setting.

  • Therapists will work as part of a comprehensive multidisciplinary team in order to provide screenings, assessments, planning, and on going services in collaborations with the child's family and other community representatives.
  • Therapists will participate in a one-on-one sessions.
  • Supports and services are conducted in a natural environment (typically home, daycare, park, etc.), in context of the family's typical routines and activities so that information is meaningful and directly relevant to supporting the child.
  • Flexible schedule, willing to work evening or weekends, as needed for service provision to families.

CONTINUING EDUCATION:

If contracted or employed as a licensed, registered, and certified professional Occupational Therapist; the practitioner shall complete the continuing educations units required to maintain professional credentialing. The Arizona Early Intervention Program provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them to build on their child's strengths, and to enhance and promote the child's growth, development and learning skills in their natural environment.

REPORTS TO: Director of Early Intervention Services
LOCATION: Services are provided in the East Valley (Mesa, Gilbert, Chandler, Tempe and East Phoenix).
SALARY: Competitive salary starting at $55 per hr.

THIS IS AN INDEPENDENT CONTRACT POSITION WITH OPTION OF BECOMING AN EMPLOYEE AND QUALIFY FOR BENEFITS.


MINIMUM QUALIFICATIONS:

  • Must possess a Maters Degree in Speech and Language Pathology at time of contract offer.

• Must have verbal and written communication skills adequate to the specific job.
• Must be at least eighteen (18) years.
• Must be over twenty-one (21) years if required to drive a RISE vehicle and meet other requirements as
 specified by the RISE Insurance Agency.
• Satisfactory background check.
• Specific language abilities (other than English) may be required to meet the needs of people/families
 receiving support, and demonstrate a knowledge of and respect for cultural differences.
• Reliable transportation to the work site and access to a telephone.
